Mini Gastric Bypass as a Revision Procedure

An educational overview of one-anastomosis (mini) gastric bypass as a revisional option after sleeve gastrectomy.

One-anastomosis gastric bypass — also known as the mini gastric bypass (OAGB) — is one of several options sometimes discussed when revising a sleeve gastrectomy for regain, inadequate loss, or selected metabolic indications.

What it is

OAGB creates a long, narrow gastric tube anastomosed to a loop of small bowel, with a single anastomosis. It produces a strong metabolic effect and is studied internationally as a primary and revisional option.

Potential benefits

  • Strong metabolic effect.
  • Single anastomosis (compared to two in Roux-en-Y gastric bypass).
  • Substantial additional weight loss in selected patients.

Considerations and risks

  • Bile reflux is a known consideration.
  • Lifelong nutritional supplementation and follow-up are essential.
  • Not appropriate for every anatomy or every patient profile.

Candidacy

OAGB as a revision is one option among several. Suitability depends on anatomy, symptoms, comorbidities, and the patient's ability to commit to long-term follow-up. Final decisions should be made with a qualified bariatric team.
