One-anastomosis gastric bypass — also known as the mini gastric bypass (OAGB) — is one of several options sometimes discussed when revising a sleeve gastrectomy for regain, inadequate loss, or selected metabolic indications.
What it is
OAGB creates a long, narrow gastric tube anastomosed to a loop of small bowel, with a single anastomosis. It produces a strong metabolic effect and is studied internationally as a primary and revisional option.
Potential benefits
- Strong metabolic effect.
- Single anastomosis (compared to two in Roux-en-Y gastric bypass).
- Substantial additional weight loss in selected patients.
Considerations and risks
- Bile reflux is a known consideration.
- Lifelong nutritional supplementation and follow-up are essential.
- Not appropriate for every anatomy or every patient profile.
Candidacy
OAGB as a revision is one option among several. Suitability depends on anatomy, symptoms, comorbidities, and the patient's ability to commit to long-term follow-up. Final decisions should be made with a qualified bariatric team.
